CHALLENGE


Ziekenhuisgroep Twente, a general teaching hospital with locations in Almelo and Hengelo, is increasingly dealing with elderly people who stay in hospital longer because there is no room available in follow-up care. Read here how we ZGT, 3 VVT institutions and Menzis have analyzed the problem and identified solutions.


APPROACH


In a period of four weeks, we interviewed more than 20 stakeholders, analyzed relevant documentation and data, and identified bottlenecks in the flow process. At the end of this period, we presented our findings and an approach to short- and long-term solutions to the directors of the organizations involved. Using these results, concrete improvement actions were initiated in 2020.
