CHALLENGE: A NEW DOOR-TO-HOME MEDICATION DISTRIBUTION MODEL
In recent years, together with the Department of Clinical Pharmacy & Toxicology of the MUMC , we have designed a new service model for the patient to make the care of the future, the care of today. Medication on Demand focuses entirely on the entire medication process from home to home.
Within the customized medication program, one of the 9 projects focuses on the distribution model of Customized Medication-Just in Time.
THE VISION: To provide outpatient and clinical integrated specialist pharmaceutical care tailored to the patient that is demonstrably safe, efficient and effective. The patient takes control and the healthcare professional offers expertise and support from a coaching role in a team of professionals.
THIS MEANS: For the clinic, the basic concept is thought of as By Use Home Medication and, where necessary, medication from the hospital pharmacy delivered Just-in-Time to the bed. By Use Home Medication means that the medication that the patient was already using at home before admission, is taken to the hospital so that they can continue to use it during their stay. Upon discharge, the patient also takes this medication home with them. For the outpatient clinic, the care surrounding medication is separated from the logistics; which makes it possible to deliver/hand over medication where and when the patient wants it.
APPROACH
1. Analysis and translation of the right data: In order to develop innovative new processes, it is necessary to take into account all influencing factors. New concepts are best developed when they are designed with the support of the right data and figures. It is important to have the right data and to place and understand it in its current context. Candide ensures that the right data is analyzed and translated into understandable information for the healthcare professional.
2. Applying new and sometimes existing/proven technology: It is of great importance. Where possible, replace manual activities with automated and digitalized process steps. This not only benefits efficiency, but also quality and safety can be better guaranteed.
3. To this end, Candide guides the implementation process in which the right technology is purchased based on fully developed business cases, resulting in a well-functioning logistics process before, during and after implementation.
An example of a new form of medication distribution in the clinic concerns the use of a medication vending machine for medication inventory management and dispensing.
An outpatient example concerns a medication collection wall, particularly for repeat prescription medication.
